Five Wishes is an advance care directive / living will that's designed to be legally binding in at least most US states, so that loved ones and health care professionals know what you want ahead of time in situations where you're unable to tell them in the moment. also covers stuff for if you die that people need to know before looking at your will. (since wills take time to process and therefore that's _not_ where one wants like what poetry to read at one's funeral.) the site and this pdf explain better.
