A while back there was a post, and I'm fairly certain it was somewhere on Dreamwidth, cataloging someone's adventures in contacting companies to discuss accessibility fail that rendered the company's website/product/location/whatever unusable for this person. I recall one of the examples of fail being a restaurant website having the menu as an image instead of text.

Does anyone happen to have a link to this post?
